Patient Information

First Visit

Plan to arrive 10 to 15 minutes early for your first appointment to fill out paperwork. If you’d like to complete your paperwork prior to your appointment, our forms can be downloaded under the “forms” tab, or click here. Please bring information about any medications currently prescribed to you. IMPORTANT: Anyone under the age of 18 must be accompanied by a parent or guardian, or given a note to perform any necessary procedures.

Scheduling

MD Family Dentistry, LLC is open Tuesday through Friday 8:30 AM until 4:00 PM. We are also open some Saturdays from 8:30 AM until 1:00 PM. We would be happy to schedule your next appointment before you leave the office to assure you get a day and time that fits your schedule.

If you have a dental emergency, we will get you in as soon as possible.

Insurance

MD Family Dentistry, LLC tries to accept most dental insurances. For a detailed list of the insurance companies with which we participate, please call the office at 410-848-8229. We do not accept and/or cannot file for any HMO dental insurances. Sorry for any inconveniences.
